Actually not his testicles but only his scrotum (the tissue sac holding the testicles); while clinically less destructive, it's no less bizarre.
Seriously, it's just a flap of skin similar to an ear lobe or belly piercing. As long as he didn't puncture any of his two testes within the sac then, barring infection, there's really no harm done to him physically. That said, he may have to undergo a lot of mental health evaluation before being let go. But on the other hand, Russia being what it is, a cop may have just pulled him to his feet, kicked him in his ass and sent him on his way, LOL...
