That picture doesn't look half bad. Most fetus in fetu pics I've seen are much nastier. In this case, the penis from the dead twin protruded from the infant's back. Most cases, the living dominant twin totally encapsulates and partially resorbs the dead twin to varying degrees. Some don't find out about this until late in life, when they have some sort of surgery and doctors discover extra bone, vestigial organ structures, or an indeterminate syncytial mass; remnants from their dead sibling.
